A simple visit workflow
Start by confirming age, then measure weight, height or length and MUAC where appropriate. Enter measurements in the calculator, compare with the chart if needed and explain the result in plain language. If a result is moderate or severe, follow local escalation guidance.

What to document
Record age in completed months, measurement date, height, weight, MUAC if used, recent illness and any visible warning signs. Clear records make follow-up easier and reduce confusion when the child is checked again.
How to explain results
Avoid blaming language. Explain that measurements are signals and that repeated monitoring helps find children who may need support. Use the chart and MUAC card as visual aids, then direct families to local health services when needed.
